Weeks 1–2
Pain
Calm the pain down.
Hands-on techniques settle the irritated cuff and bursa and take the edge off, starting your very first visit, so you can sleep on that side again.
What We Treat
The jolt when you reach for a seatbelt. The night you can’t lie on that side. The paddle that used to be automatic. Your shoulder is the most mobile joint you own, and that’s exactly why it needs a cause-first plan, not another guess.

Why It Happens
Your shoulder trades stability for range, it moves in more directions than any other joint, and it leans on the rotator cuff and the shoulder blade to stay centered while it does. When that system gets out of sync, pain follows. It’s usually a mix of a few culprits: a rotator cuff that got weak or irritated, a shoulder blade that stopped gliding the way it should, a stiff upper back and posture that crowd the joint, and overhead load, paddling, pressing, throwing, that keeps reloading the same angry spot before it settles.
None of that shows up in a two-minute glance or a single X-ray. It takes a real, cause-first evaluation to find which link in the chain is actually driving your pain. That’s why rest, a sling, or a generic band routine so often stalls out: they chase the ache instead of the cause. Find the real driver, and the plan writes itself.
The Honest Truth
Nobody regrets starting too early. Every week, someone in our clinic says they regret waiting.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for a shoulder.
Weeks 1–2
Pain
Hands-on techniques settle the irritated cuff and bursa and take the edge off, starting your very first visit, so you can sleep on that side again.
Weeks 2–6
Prime
Targeted mobility work restores the overhead and reaching range the joint has been guarding, and gets the shoulder blade gliding again.
Weeks 6–10
Perform
Progressive rotator cuff and scapular strengthening rebuilds the stability the joint was missing, so overhead stops feeling like a gamble.
Weeks 10+
Prevent
Paddling, pressing, and throwing mechanics get tested under real load before you’re cleared to go all-in: that’s the endpoint, not just another appointment.
One injury to my right shoulder completely altered my life forever, but we have the results that will allow me to maintain somewhat normal function of my right arm until eventually having shoulder replacement surgery down the road. Without the proper therapy it would’ve been more devastating.

Questions, Answered
Usually not, and neither is the first step. Plenty of pain-free shoulders show cuff fraying and “wear and tear” on an MRI, so a scan doesn’t always point to the real cause, and a cortisone shot can quiet the pain without fixing why it started. We begin with a hands-on evaluation to find what’s actually driving your symptoms, and if imaging or an injection ever becomes genuinely useful, we’ll help you get it. You don’t have to wait on a scan to start feeling better.
Most shoulder pain is not a full tear, and the large majority never needs surgery. Even many diagnosed rotator cuff tears respond well to a targeted strengthening plan and never see an operating room. What matters is matching the plan to your specific shoulder, which is exactly what a Doctor of PT sorts out at your evaluation. If your case is one of the few that truly needs a surgeon, we’ll tell you early and help you get there, instead of letting months slip by.
Most patients feel meaningful change within 4–6 visits. A full plan for a shoulder runs through all four phases, Pain, Prime, Perform, and Prevent, typically over 8–12 weeks, depending on how long you’ve been dealing with it and whether it’s a stiffness or a strength problem. Either way, you get a plan with an endpoint: you’ll always know which phase you’re in and what’s left, not an open-ended string of appointments.
